-Providing medical case management for seniors and people with chronic health conditions;

Transporting guests to and from medical appointments

Accompanying guests to doctor visits

Communicating with guest's medical providers

Paying for, picking up and delivering medications to our guests


-Partnering with Atlantic Health Systems (AHS);

Working with AHS when a patient is identified as experiencing homelessness and managing the patient’s discharge

Working with AHS when a patient experiencing homelessness is diagnosed with COVID-19, arranging for a safe place for the patient to recover and providing assistance while the patient recovers

—Increasing our Outreach teams ability to make daily, weekly, and monthly check in calls to guests that exhibit symptoms of depression, anxiety and other mental health conditions


-Connecting individuals to substance abuse prevention programs or inpatient mental health services, when a more intensive level of care is required


-Offering free, 1:1 counseling sessions for guests without access to appropriate mental health care
